*Jamboree format, no playoffs.

  • Submit payment online via credit card or electronic check or mail a check, payable to Achieve Lacrosse.
  • You will be able to register multiple teams, provide team names, and indicate the level of play for each of your teams.
  • Complete registration by emailing team rosters to Carla (Gigon) Farkes @ carla.farkes@gmail.com
  • Bring player waivers with you on the day of the tournament.

*Tournament roster forms and player waivers must be completed and submitted.  Team rosters should be emailed to Carla (Gigon) Farkes at carla.farkes@gmail.com using the above template. 
**Achieve Lacrosse subscribes to Bollinger Insurance to provide additional insurance coverage for all training program, league and tournament participants.  Achieve Lacrosse requires each individual player to complete a player waiver prior to participation in any event.  Electronic waivers are included in our online registration for training programs and leagues.  If accurate information is not submitted through the online registration, secondary insurance coverage CANNOT and will not be provided.
For tournaments:  In addition to player waivers for each team member, Captains/Coaches/Directors must submit an Excel document of the COMPLETED roster prior to the start of the tournament.  Rosters MUST follow the Bollinger template in order for the athletes/team to receive secondary insurance coverage.  The template includes the following fields: first name, last name, emergency contact number, age, team name, and address (including city, state and zip code.)  If an accurate roster is not submitted to the tournament director in advance of the tournament, secondary insurance coverage CANNOT and will not be provided.  (A hard copy of the completed tournament roster should also be turned in to the tournament director on the day of the event so that it is on hand for the athletic trainer(s) in the event of an emergency.)
